Your Appointment

Appointments can be made by contacting our office.

You will be sent a confirmation letter and a patient registration sheet to complete prior to your appointment.

Please bring the following to your appointment:

1. Referral letter

2. Completed patient registration sheet

3. Medicare &/or DVA cards

4. Private health insurance card

5. All x-rays or scans that you have

6. A list of your current medications


Payment for all consultations is required on the day of appointment.

Please contact our office if you require a quote for your consultation.

We can accept cash, EFTPOS, credit card payments, bank cheques or money orders. Personal cheques are not accepted.

We can submit your Medicare claim electronically if you know your bank account details for reimbursement.


If you are unable to attend your appointment you must notify our office as soon as possible. We reserve the right to charge a cancellation fee and/or refuse to make further bookings if you do not attend your appointment(s) and fail to provide adequate notification.
